NEW PATIENTS
To establish a new cryostorage account for your reproductive specimens, you will begin by completing ReproTech account forms.
Please respond to the question(s) below. You will then be taken to the DocuSign site to begin the forms process. If you are storing more than one specimen type, please complete forms for each type of specimen. Forms completed in DocuSign will automatically be sent to ReproTech and you will receive a copy via email.
We recommend you use an email address that is private for accepting emails that will contain Personal Health Information.
ReproTech has partnered with Delaware Institute for Reproductive Medicine to waive the transfer fee for one shipping tank to ReproTech if your specimens are included in a group shipment. You also have the option to ship your specimens in two shipping tanks which would incur a fee of $375.
ReproTech will keep your forms on file, and your storage account will be activated closer to the time when your specimens will be shipped for storage, depending on your treatment plan. Once ReproTech has received your specimens, you will be notified.
